Eligibility Criteria for Uttar Pradesh Rastriya Parivarik Labh Yojana

- Must be a permanent resident of the state Uttar Pradesh
- Applicant family must belong to the Below Poverty Line
- Any event of death of the chief earner that is the family head of the family would make the family eligible for the scheme. In case of death any other member of such family will not be eligible for parivarik labh yojana
- The financial benefit would be given only to those families whose family head is between 18 to 60 years
- Annual earning of the whole family in the urban area must not exceed Rs 56,000
- Total earning of the entire family living in rural area must not exceed Rs 46,000
- Applicant need to apply within one year of from the date of death of family head
- Any eligible rural or urban poor household must necessarily hold a bank account
- Family benefit will be paid to the household (includes spouse, minor children, unmarried daughter and dependent parents) of the deceased person

- Application form needs to be filled in English
- National-level bank account details needs to be provided by the applicant
- Under this scheme Co-operative bank account is not valid
- Income certificates issued by the Tehsil are only valid
- Applicants will be held responsible for any error or misleading entries made in the application form
- Shadow copy of all the documents are needed to be uploaded, while filling the parivarik labh yojana online form
- Recognized hospitals, nagar panchayat or tehsil hospitals are eligible to issue the death certificate of the deceased family member
- Passport size photographs and signature of the applicant must be less than 20KB in JEPG format
- Details of the beneficiary including bank passbook, death certificate of the deceased, etc must be in PDF format and should not exceed 20KV
